Join today by completing a registration
form and signing up for the direct payment plan. Download
the PDF form to the left, complete the form and stop by
HealthWorks to become a member.
Activation Fee
An activation fee* of $50 is due at
the time of registration along with the first month of
membership dues.
Buddy Up! By signing
up with a buddy, the activation fee is split in half
making your activation fee only $25.
Membership Dues
Monthly membership dues are $35.00
plus tax.
Health Club Reimbursement
HealthWorks partners with Blue Cross/Blue
Shield offering $20 per month based on 12 visits per month
to qualified members. In addition, HealthWorks pays members
who belong to Medica and Health Partners $20 per month
based on 8 or 12 visits per month respectively. We hope
to have a formal agreement in place for Medica and Health
Partners in the very near future.
